Wine Club Terms

Wining with Us is an Enjoyable *Experience* But..............

 

First Please Review Our Agreement Terms:

 

By joining the Wine Club, I certify that I am, and the recipient of this gift is, at least 21 years of age.

 

I agree that I will receive my Wine Club shipments in accordance with the My Hearty Leaf Tea & Wine Boutique schedule for the clubs that I have joined.

 

I understand that all memberships are for a minimum of one month (1 shipment), with the option of extending my subscription from 1 month to 3 months and even 6 months.  If I decide to do so, I understand that I have to contact My Hearty Leaf Tea & Wine Boutique at least 7 business days before my current subscription payment is due.  If I do not contact My Hearty Leaf Tea & Wine Boutique in the required time frame as listed above.

 

I understand that I risk the chance of my card on file being charged if changes are not completed within 7 business days of my subscription's due date.

 

I may cancel any membership after one month if notice is received in writing at least 15 days prior to scheduled shipment. An early cancellation fee will be applied if there is a request to cancel before the commitment is met.

 

I understand that I may skip 1 club shipment if needed, after receiving a minimum of 2 consecutive shipments.

 

I hereby authorize My Hearty Leaf Tea & Wine Boutique to charge my credit card 5 business days prior to each shipment of wine.

 

I understand and accept that pricing is subject to change based upon the following methodology: 10% off My Hearty Leaf Tea & Wine Boutique retail price for selected wines at the time of shipment.

I am aware that the government requires wine shipments to be signed for at time of delivery by an adult over the age of 21.

 

I understand that I am responsible for all costs incurred if my wine shipment is undeliverable due to outdated or inaccurate contact information.
